Herbs for men
Horny goat weed benefits: does it help erections and libido?
Horny goat weed, or epimedium, appears in many “male enhancement” products. Its catchy name comes from a legend about goats becoming more active after grazing on it. Here is what we know about horny goat weed benefits for men.
In short: Horny goat weed contains icariin, a compound that in laboratory and animal studies acts on the same enzyme as some ED medicines. However, there are very few good human trials, so its effect on erections in men is unproven. It may interact with blood pressure and blood-thinning medicines, and high doses can cause side effects.
What is horny goat weed?
Horny goat weed is the common name for several *Epimedium* species, used in traditional Chinese medicine for fatigue, joint pain and low sexual desire.
How it might work
The main active compound, icariin, has been shown in laboratory studies to weakly block PDE5, the enzyme that prescription ED tablets target. Animal studies have found improved erectile function. Icariin may also support nitric oxide release.
What the evidence in men shows
- Good-quality human trials for erectile dysfunction are lacking.
- The amount of icariin in commercial products varies hugely, and many contain very little.
- Effects seen in animals do not always translate to people.
So while horny goat weed is an interesting herb, it should not be relied on as an ED treatment.
Safety concerns
- Side effects reported include dizziness, dry mouth, nosebleeds, irritability and, at high doses, rapid heartbeat.
- It may lower blood pressure and slow blood clotting, so avoid combining it with blood pressure medicine, blood thinners or ED medicine without advice.
- Many “horny goat weed” blends sold online also contain undeclared prescription drugs. SAHPRA and the US FDA have warned about such products.
A smarter approach
If you are drawn to horny goat weed because erections have weakened, start with the factors that make the biggest difference: exercise, healthy weight, quitting smoking and less alcohol. Herbs with more research, such as Korean red ginseng, may be worth discussing. See ginseng for erectile dysfunction and home remedies for erectile dysfunction.

Private self-check: 5 quick questions
Answer these honestly before you try anything new or book. They help you describe what is happening and get more from your first consultation.
- How often can you get and keep an erection firm enough for sex: almost always, most of the time, sometimes, or rarely?
- Do you still notice morning or night-time erections?
- Have stress, poor sleep, alcohol, smoking or long hours increased recently?
- Do you take medicine for blood pressure, the heart, diabetes or mood?
- Has your interest in sex dropped noticeably over the past year?
If you answered “sometimes” or “rarely” to the first question, a consultation is a sensible next step. Morning erections that still happen are often a reassuring sign that lifestyle, stress or confidence are playing a part, which natural support can address well. A yes to question four means any remedy needs checking for interactions first.
