Erectile dysfunction after prostate surgery: recovery and support
Erection changes are common after prostate cancer surgery and some other prostate procedures. Knowing what to expect about erectile dysfunction after prostate surgery helps you plan your recovery.
In short: After radical prostatectomy, most men experience erection problems because the nerves controlling erections are stretched or damaged. Recovery can take 12 to 24 months or longer and depends on age, pre-surgery erections and whether nerves were spared. Penile rehabilitation, pelvic floor training and heart-healthy habits support recovery.
Why surgery affects erections
The nerves that trigger erections run alongside the prostate. During surgery they may be stretched, bruised or removed. Nerve-sparing techniques improve the chances of recovery, but healing takes time.
Other changes
- Dry orgasm: the prostate and seminal vesicles are removed, so there is no semen. Orgasm is still possible. See dry ejaculation treatment.
- Length change: some men notice a slight shortening. See why is my penis getting smaller.
- Leakage: some men leak urine during arousal or orgasm, which usually improves.
Recovery timeline
Erections may start returning within months, but recovery often continues for up to two years or longer. Men who had good erections before surgery and nerve-sparing procedures tend to recover more.
Penile rehabilitation
Many urology teams recommend “penile rehabilitation”: encouraging regular blood flow to the penis after surgery using medicines or devices prescribed by your medical team, to protect the tissue while nerves heal. Follow your team’s advice.
Natural steps that support recovery
- Pelvic floor training: helps both continence and erections. See Kegel exercises for men.
- Exercise: supports blood flow and mood. See exercises for erectile dysfunction.
- Heart-healthy diet and no smoking.
- Patience and communication with your partner. See how to talk to your partner about erectile dysfunction.
- Emotional support: a cancer diagnosis is a lot to process. CANSA offers support.

Private self-check: 5 quick questions
Answer these honestly before you try anything new or book. They help you describe what is happening and get more from your first consultation.
- How often can you get and keep an erection firm enough for sex: almost always, most of the time, sometimes, or rarely?
- Do you still notice morning or night-time erections?
- Have stress, poor sleep, alcohol, smoking or long hours increased recently?
- Do you take medicine for blood pressure, the heart, diabetes or mood?
- Has your interest in sex dropped noticeably over the past year?
If you answered “sometimes” or “rarely” to the first question, a consultation is a sensible next step. Morning erections that still happen are often a reassuring sign that lifestyle, stress or confidence are playing a part, which natural support can address well. A yes to question four means any remedy needs checking for interactions first.
