Pelvic floor dysfunction in men: tight muscles, pain and sexual health

Most men only hear about the pelvic floor in the context of Kegels, but the muscles can be too tight as well as too weak. Pelvic floor dysfunction in men can cause pain, urinary problems and sexual difficulties.

In short: Pelvic floor dysfunction means the muscles at the base of the pelvis are too tight, too weak or poorly coordinated. Tight (overactive) muscles can cause pelvic, groin or testicle pain, pain with ejaculation, urinary hesitancy and erection problems. Weak muscles can cause leakage and weaker erections. Treatment focuses on relaxation, coordination and, when appropriate, strengthening.

What does the pelvic floor do?

The pelvic floor muscles support the bladder and bowel, control urination and bowel movements, and play key roles in erection and ejaculation.

Types of dysfunction

Overactive (tight) pelvic floor

Muscles stay tense, often due to stress, long sitting, heavy lifting, holding urine, or past pain. Signs include:

  • Pain in the perineum, groin, testicles, penis or lower back
  • Pain during or after ejaculation. See painful ejaculation causes.
  • Difficulty starting urination or a weak stream
  • Constipation or pain with bowel movements
  • Erection problems or premature ejaculation

Underactive (weak) pelvic floor

Signs include urine dribbling, leakage after prostate surgery and weaker erections. Kegels help here. See Kegel exercises for men.

Why it matters to know the difference

Doing Kegels when your muscles are already tight can make pain worse. If you have pelvic pain, focus on relaxation first.

Natural ways to relax a tight pelvic floor

  • Diaphragmatic breathing: breathe deeply into your belly and feel the pelvic floor gently drop as you inhale.
  • Reverse Kegels: gently lengthen and relax the pelvic floor as if starting to urinate.
  • Stretching: child’s pose, happy baby, deep squat and hip flexor stretches.
  • Warm baths to ease muscle tension.
  • Break up long sitting every 30 to 45 minutes. See cycling and erectile dysfunction.
  • Manage stress, which often shows up as pelvic tension. See performance anxiety in men.
  • Avoid straining on the toilet; eat fibre and drink water.

When to see someone

Get checked by an appropriately registered medical professional if you have blood in your urine or semen, fever, discharge, or severe pain, to rule out infection or other causes. A pelvic health physiotherapist can assess and guide treatment.
