Epididymitis is one of the most common causes of testicle pain and swelling in adult men. Here is what to know about epididymitis, from causes to recovery.

In short: Epididymitis is inflammation of the epididymis, the coiled tube behind the testicle that stores sperm. It is usually caused by a bacterial infection, often an STI in younger men or a urinary infection in older men. It causes pain, swelling and warmth that develop over a day or two. It needs medical assessment and treatment.

Causes

  • Sexually transmitted infections such as chlamydia and gonorrhoea, especially in men under 35.
  • Urinary tract infections, more common in older men or with an enlarged prostate. See UTI in men.
  • Recent catheter or urological procedures.
  • Non-infectious causes, including heavy lifting with a full bladder, some medicines and injury.

Symptoms

  • Pain and tenderness in one testicle, usually developing gradually
  • Swelling and warmth of the scrotum
  • Pain when urinating or more frequent urination
  • Discharge from the penis
  • Fever in some cases

Epididymitis vs torsion

Torsion causes sudden, severe pain and is an emergency. Epididymitis pain usually builds over hours to days. If in doubt, seek urgent care. See testicular pain causes.

Treatment

An appropriately registered medical professional will test for infection and prescribe appropriate antibiotics. If an STI is the cause, partners also need testing and treatment.

Recovery tips

  • Rest and elevate the scrotum.
  • Use a cold pack wrapped in a cloth.
  • Wear supportive underwear.
  • Avoid sex until treatment is complete and symptoms settle.
  • Pain usually improves within days, but swelling can take weeks.

Prevention

  • Use condoms. See STI symptoms in men.
  • Treat urinary symptoms early.
  • Empty your bladder before heavy lifting.

  1. Do you have pain, swelling, redness or itching in the genital area?
  2. Have you noticed discharge, sores, a lump or blood in your urine or semen?
  3. Do you have a fever or feel generally unwell?
  4. Has this lasted more than a few days, or keep coming back?
  5. Is it affecting your sex life, sleep or confidence?

A yes to questions two or three, or sudden severe pain, means you should see an appropriately registered medical professional promptly. For recurring or lifestyle-related concerns, a naturopath can help with natural support alongside medical care.

Your health and safety

Our naturopaths use natural remedies and lifestyle support, and always check your health history and current medicines first. Epididymitis usually needs testing and medical treatment, so we refer you to an appropriately registered medical professional promptly. Avoid unregistered “enhancement” products sold online or at markets, as SAHPRA has warned that some contain hidden ingredients.

Seek urgent medical care for chest pain, an erection lasting more than four hours, sudden weakness or other sudden symptoms.

A consultation is R2,500, in person or remote, and your naturopath confirms any further costs before you go ahead. See our men’s clinic prices for details.
