STI symptoms in men: signs to watch for and why testing matters
Sexually transmitted infections are common and treatable, but many cause no symptoms at all. Knowing the STI symptoms in men, and getting tested regularly, protects you and your partners.
In short: Common STI symptoms in men include discharge from the penis, burning when urinating, sores, blisters, warts or bumps, rashes, and testicle pain or swelling. Many STIs, including chlamydia, often cause no symptoms. Regular testing, condoms and prompt treatment for you and your partners are essential. South African public clinics offer free STI services.
Common symptoms
- Discharge from the penis (white, yellow or green)
- Burning or pain when urinating
- Sores, ulcers or blisters on the penis, anus or mouth

- Testicle pain or swelling. See epididymitis.
- Rash on the body, palms or soles (possible syphilis)
- Flu-like symptoms after exposure (possible early HIV)
Common STIs
- Chlamydia and gonorrhoea: discharge and burning, or no symptoms.
- Syphilis: painless ulcer, later rash.
- Herpes: painful blisters.
- HPV: genital warts.
- Trichomoniasis: mild irritation or discharge.

Why testing matters
Many STIs have no symptoms but can still cause complications such as epididymitis, infertility and passing infection to partners. Test if you have a new partner, multiple partners, symptoms, or a partner with an STI.

Public clinics in South Africa offer free STI and HIV testing and treatment. Private practitioners and sexual health clinics also test. Our practice does not test for or treat STIs, but we can refer you.

STIs need testing and medical treatment, so we refer you to an appropriately registered medical professional or clinic.
