PSA test explained: what your prostate blood test means

The PSA test is the most common blood test for prostate health, yet many men are unsure what it measures. Here is the PSA test explained in plain language.

In short: PSA (prostate-specific antigen) is a protein made by the prostate. A blood test measures its level. Raised PSA can be a sign of prostate cancer, but it is also raised by an enlarged prostate, prostatitis, urinary infection, recent ejaculation or vigorous cycling. A PSA test is a starting point, not a diagnosis, and it makes sense to decide on testing together with a medical professional.

What is PSA?

Prostate-specific antigen is a protein produced by the prostate gland. Small amounts enter the blood.

What can raise PSA?

Pros and cons of PSA testing

Pros: it can detect prostate cancer early, before symptoms.

Cons: it can be raised without cancer (false alarms), can miss some cancers, and may lead to further tests and treatment of slow-growing cancers that might never cause harm.

Who should consider testing?

Guidelines vary, but men often discuss testing from around age 50, or from 40 to 45 if they have a family history of prostate cancer or are of African descent, as risk is higher. CANSA provides information for South African men.

Before your test

  • Avoid ejaculation for 48 hours.
  • Avoid vigorous cycling or exercise for 48 hours.
  • Tell the tester about any urinary infection or recent procedures.

What happens after a raised PSA?

Usually a repeat test, examination, and possibly an MRI scan or biopsy. A single raised result is not a diagnosis.

Warning signs not to ignore

Blood in urine or semen, difficulty urinating, bone pain or unexplained weight loss need prompt assessment. See blood in semen.
