Blood in semen: common causes and when to get checked

Seeing blood in semen is frightening, but in younger men it is usually caused by something minor and settles on its own. It is still important to know when it needs checking.

In short: Blood in semen, called haematospermia, makes semen look red, pink or brown. In men under 40 it is often caused by minor inflammation, a recent prostate procedure or vigorous sexual activity, and usually clears within weeks. It should be checked if it keeps happening, you are over 40, or you have pain, fever, urinary symptoms or blood in the urine.

Common causes

  • Inflammation or infection of the prostate, seminal vesicles, urethra or epididymis. See painful ejaculation causes.
  • Recent procedures such as a prostate biopsy or vasectomy.
  • Vigorous or prolonged sexual activity or long periods without ejaculation.
  • Sexually transmitted infections.
  • High blood pressure in some men.
  • Blood-thinning medicines.

Less common causes

  • Prostate enlargement or stones
  • Blockages in the ejaculatory ducts
  • Rarely, prostate, bladder or testicular cancer, especially in older men

When to get checked

See an appropriately registered medical professional if:

  • You are over 40
  • It happens more than once or lasts longer than a few weeks
  • You have pain, fever, or burning when urinating
  • You also have blood in your urine
  • You have a lump in the testicle
  • You have a history of cancer or bleeding disorders

Assessment may include a urine test, STI tests, a prostate examination, PSA blood test and blood pressure check.

While you wait

  • Avoid alcohol and spicy food if you have urinary irritation.
  • Stay hydrated.
  • Avoid strenuous activity if you have pelvic pain.
  • Practise safer sex until infection is ruled out.

Recovery

Most young men with a single episode recover fully without treatment. Infections are treated with appropriate medicines.

Your health and safety

Our naturopaths use natural remedies and lifestyle support, and always check your health history and current medicines first. Blood in the semen needs medical assessment in many cases, so we refer you to an appropriately registered medical professional for tests whenever appropriate. Avoid unregistered “enhancement” products sold online or at markets, as SAHPRA has warned that some contain hidden ingredients.

Seek urgent medical care for chest pain, an erection lasting more than four hours, sudden weakness or other sudden symptoms.

Consultation price

A consultation is R2,500, in person or remote, and your naturopath confirms any further costs before you go ahead. See our men’s clinic prices for details.
