Blue balls: what causes it, is it harmful, and how to relieve it
“Blue balls” is the slang name for an ache in the testicles after sexual arousal without ejaculation. It is real, common and usually harmless. Here is what causes it and how to ease it.
In short: Blue balls, known medically as epididymal hypertension, is a temporary ache or heaviness in the testicles after prolonged arousal without orgasm. Blood builds up in the genitals and takes time to drain. It is not harmful and usually settles within an hour or two. Severe, one-sided or persistent pain needs checking.
What causes blue balls?
During arousal, blood flows into the penis and testicles and the testicles draw up closer to the body. If arousal lasts a long time without ejaculation, the extra blood and pressure can cause a dull ache. The term “blue” refers to a slight bluish tinge that is rarely noticeable.
Symptoms
- Aching or heaviness in the testicles
- Mild discomfort in the groin or lower abdomen
- Symptoms start after prolonged arousal and fade as arousal subsides
Is it harmful?
No. Blue balls does not damage the testicles or affect fertility. It is also never a reason to pressure a partner into sexual activity. Discomfort settles on its own.
How to relieve blue balls
- Wait it out: the ache usually fades within an hour or two.
- Distract yourself and let arousal subside naturally.
- Exercise or walk to redirect blood flow.
- A cool shower or cold pack wrapped in cloth can help.
- Ejaculation, through masturbation, relieves pressure quickly for some men.
- Supportive underwear may ease the heaviness.
When it is not blue balls
See an appropriately registered medical professional or emergency care if you have:
- Sudden, severe pain, especially in one testicle (possible torsion, an emergency)
- Swelling, redness or warmth
- Fever or pain when urinating
- A lump in the testicle
- Pain that lasts more than a few hours or keeps recurring without arousal
Frequent discomfort
If you regularly have pelvic or testicle discomfort, pelvic floor tension may play a role. See pelvic floor dysfunction in men.
