In short: HPV in men is extremely common and usually causes no symptoms, clearing on its own within a year or two. Some types cause genital warts, and a few high-risk types are linked to cancers of the penis, anus and throat. Vaccination and condoms reduce risk.

What is HPV?

Human papillomavirus (HPV) is a group of more than 100 related viruses. Around 40 types can affect the genital area. Most sexually active people will have HPV at some point in their lives.

How HPV spreads

HPV spreads through skin-to-skin contact during vaginal, anal or oral sex. It can spread even when there are no visible signs. Condoms reduce but don’t fully prevent spread, because HPV can be on skin not covered.

Symptoms in men

Most men have no symptoms, and the immune system clears the virus naturally. When symptoms do occur:

  • Genital warts: soft, flesh-coloured bumps on the penis, scrotum, groin or anus, sometimes in a cauliflower shape. See genital warts treatment in males.
  • Rarely, cancer: persistent high-risk HPV is linked to cancers of the penis, anus and back of the throat. These are uncommon but worth knowing about.

Testing for men

There is no routine HPV test for men. Genital warts are diagnosed by appearance. Men with concerns about anal or throat changes should speak to an appropriately registered medical professional.

Signs that need checking

  • New bumps or warts on the genitals or anus
  • A sore, lump or colour change on the penis that doesn’t heal
  • Persistent sore throat, difficulty swallowing or a lump in the neck
  • Anal bleeding, pain or itching that doesn’t settle

Prevention

  • HPV vaccination: most effective before sexual activity begins, and also recommended for some adults. South Africa’s school programme vaccinates girls; boys and men can ask about private vaccination.
  • Condoms reduce risk
  • Fewer sexual partners lowers exposure
  • Not smoking helps the immune system clear HPV

HPV and relationships

An HPV diagnosis in a partner doesn’t mean anyone has been unfaithful – the virus can stay silent for years. Calm, honest conversations help. See how to improve intimacy in a relationship.

Other bumps that are harmless

Many bumps on the penis are normal, such as pearly penile papules and Fordyce spots.

  1. Do you have pain, swelling, redness or itching in the genital area?
  2. Have you noticed discharge, sores, a lump or blood in your urine or semen?
  3. Do you have a fever or feel generally unwell?
  4. Has this lasted more than a few days, or keep coming back?
  5. Is it affecting your sex life, sleep or confidence?

A yes to questions two or three, or sudden severe pain, means you should see an appropriately registered medical professional promptly. For recurring or lifestyle-related concerns, a naturopath can help with natural support alongside medical care.
