Penile fracture: signs, causes and why it is an emergency
A penile fracture sounds alarming, and it is a genuine emergency. It is uncommon, but knowing the signs means you can get help fast and protect your future erections.
In short: A penile fracture is a tear in the tunica albuginea, the tough layer around the erection chambers, usually from forceful bending of an erect penis during sex. Signs include a popping or cracking sound, sudden pain, rapid loss of erection, and swelling with dark bruising. It needs emergency surgical repair, ideally within hours.
What happens in a penile fracture?
The penis has no bone. During an erection, two spongy chambers (corpora cavernosa) fill with blood under pressure, wrapped in a tough sheath called the tunica albuginea. If the erect penis is bent forcefully, this sheath can tear, causing bleeding into the tissues.
Signs of a penile fracture
- A popping, cracking or snapping sound
- Sudden, sharp pain
- Immediate loss of erection
- Rapid swelling and dark bruising, sometimes called an “eggplant deformity”
- The penis may bend to one side
- Blood at the tip or difficulty urinating can mean the urethra is also injured
Common causes
- Vigorous intercourse where the penis slips out and hits the partner’s pelvis
- Partner-on-top positions with downward force
- Forceful bending or rolling over onto an erection
- Practices in some cultures of “cracking” an erection to make it go down
What to do
Go to an emergency department immediately. Do not wait to see if it settles. Early surgical repair, usually within 24 hours, gives the strongest chance of preserving erectile function and preventing curvature.
Possible long-term effects
Without prompt treatment, complications can include:
- Erectile dysfunction

- Painful erections

- Slow down and communicate during vigorous sex.
- Take care with positions where your partner is on top.
- Never forcefully bend an erection.
