A torn frenulum is a common and alarming injury because it can bleed a lot. Here is what to know about frenulum breve and foreskin tears.

In short: The frenulum is the small band of tissue on the underside of the penis that connects the foreskin to the head. A short frenulum (frenulum breve) can cause pain, tightness and tearing during erections or sex. Tears usually bleed heavily at first but heal within a couple of weeks. Repeated tears may need a minor procedure.

What is the frenulum?

The frenulum is a fold of skin under the head of the penis. It contains a small artery, which is why tears can bleed significantly.

What is frenulum breve?

When the frenulum is short, it can pull the head of the penis downwards during an erection and make retracting the foreskin painful.

Signs

  • Pain or tightness under the head during erections or sex
  • The head bending downwards
  • Difficulty pulling back the foreskin
  • Tearing and bleeding during sex or masturbation

First aid for a frenulum tear

1. Stay calm. Bleeding often looks worse than it is.

2. Apply firm pressure with a clean cloth for 10 to 15 minutes.

3. If bleeding does not stop after 20 minutes of pressure, go to an emergency department.

4. Keep the area clean with warm water.

Healing

  • Most tears heal within one to two weeks.
  • Avoid sex and masturbation until fully healed.
  • Use plenty of lubricant when you resume.
  • Scar tissue can make re-tearing more likely.

Prevention

  • Use lubricant during sex.
  • Avoid very vigorous movements.
  • Gently stretch the foreskin in the shower if advised.

When to see a medical professional

  • Bleeding that will not stop
  • Repeated tears
  • Ongoing pain or tightness

A simple procedure called frenuloplasty can lengthen the frenulum. See also phimosis: tight foreskin in adult men and pain during erection.
