In short: Painful erections are commonly caused by Peyronie’s disease, a tight foreskin (phimosis), a short or torn frenulum, infection or inflammation, or injury. An erection lasting more than four hours (priapism) is a medical emergency. Painful erections with a new bend, lump or swelling should be assessed.

Common causes

Peyronie’s disease

Scar tissue in the penis can cause pain, especially in the early phase, along with a bend or lump. See is a curved penis normal.

Tight foreskin (phimosis)

A foreskin that is too tight can pull painfully during erections. See phimosis: tight foreskin in adult men.

Short or torn frenulum

The band of tissue under the head of the penis can be too short or tear during sex. See frenulum breve and foreskin tears.

Infection and inflammation

Balanitis, urinary infections or prostatitis can make erections uncomfortable. See balanitis and prostatitis natural remedies.

Injury

A recent knock or rough sex can cause bruising. A popping sound with sudden pain and swelling is an emergency. See penile fracture.

Priapism

An erection lasting more than four hours, often painful, that is not related to arousal. It is a medical emergency.

Sleep-related painful erections

A rare condition causing painful erections during sleep.

Your health and safety

Our naturopaths use natural remedies and lifestyle support, and always check your health history and current medicines first. Painful or prolonged erections can be emergencies, so we always direct you to urgent care when symptoms suggest priapism or injury. Avoid unregistered “enhancement” products sold online or at markets, as SAHPRA has warned that some contain hidden ingredients.

Seek urgent medical care for chest pain, an erection lasting more than four hours, sudden weakness or other sudden symptoms.
