Why do testicles hang low? What is normal and when to get checked
Many men notice their testicles hanging lower than before, or one lower than the other. Here is why testicles hang low, and when it is worth getting checked.
In short: Testicles hang outside the body to stay a few degrees cooler, which helps sperm production. The scrotum relaxes in heat and tightens in cold. It is normal for one testicle (usually the left) to hang lower, and for the scrotum to sag more with age. New swelling, heaviness, a lump or pain should be checked.
Why testicles hang
Sperm develop well a few degrees below body temperature. The cremaster muscle and the skin of the scrotum adjust the testicles’ position to control temperature:
- Heat: muscles relax and testicles hang lower.
- Cold or arousal: muscles tighten and testicles draw up.
Why one hangs lower
It is very common for one testicle, usually the left, to hang slightly lower. This may relate to differences in the veins and cord length.
Age and sagging
With age, skin loses elasticity and the cremaster muscle weakens, so the scrotum may hang lower. This is normal.
When low-hanging testicles may need checking
- Varicocele: enlarged veins that feel like a bag of worms and cause a dragging ache. See varicocele.
- Hydrocele: fluid around the testicle causing painless swelling. See hydrocele in men.
- Hernia: a bulge in the groin that may extend into the scrotum.
- Testicular lump: always check. See testicular self-exam.
See an appropriately registered medical professional if you notice swelling, heaviness, a lump, or pain. Sudden severe pain is an emergency. See testicular pain causes.
