Testicular self-exam: how to check your testicles in 2 minutes

Testicular cancer is one of the most common cancers in young men, and it is highly treatable when found early. A monthly testicular self-exam takes two minutes and helps you notice changes quickly.

In short: Check your testicles once a month, ideally after a warm shower when the scrotum is relaxed. Gently roll each testicle between your thumb and fingers, feeling for lumps, swelling, hardness or changes in size. It is normal for one testicle to be slightly larger or hang lower. See an appropriately registered medical professional promptly about any new lump or change.

Why self-checks matter

Testicular cancer most often affects men aged 15 to 49. Most cases are found by men themselves as a painless lump or swelling. Early detection makes treatment simpler and very successful.

How to check, step by step

1. Choose the right time: after a warm bath or shower, when the scrotum is relaxed.

2. Stand in front of a mirror: look for swelling or changes in the skin.

3. Check one testicle at a time: support it in your palm and gently roll it between your thumb and fingers.

4. Feel the surface: it should feel smooth and firm, like a hard-boiled egg without its shell.

5. Find the epididymis: the soft, tube-like structure at the back and top of each testicle is normal and should not be mistaken for a lump.

6. Repeat on the other side.

What is normal

  • One testicle slightly larger or hanging lower than the other
  • The epididymis at the back
  • Blood vessels and tubes above the testicle

What to look for

  • A lump or hard area, even if painless
  • Swelling or enlargement
  • A change in shape or firmness
  • A feeling of heaviness in the scrotum
  • A dull ache in the lower abdomen or groin
  • Sudden fluid build-up in the scrotum

What to do if you find something

Do not panic, as most lumps are not cancer. Causes include cysts, varicocele and infection. See varicocele and testicular pain causes. But do see an appropriately registered medical professional promptly. An ultrasound scan is usually quick and painless.

Sudden severe pain

Sudden severe testicle pain may be torsion, an emergency. Go to emergency care immediately.

  1. Do you have pain, swelling, redness or itching in the genital area?
  2. Have you noticed discharge, sores, a lump or blood in your urine or semen?
  3. Do you have a fever or feel generally unwell?
  4. Has this lasted more than a few days, or keep coming back?
  5. Is it affecting your sex life, sleep or confidence?

A yes to questions two or three, or sudden severe pain, means you should see an appropriately registered medical professional promptly. For recurring or lifestyle-related concerns, a naturopath can help with natural support alongside medical care.

Your health and safety

Our naturopaths use natural remedies and lifestyle support, and always check your health history and current medicines first. Any new lump or change in a testicle needs medical assessment, so we always refer you to an appropriately registered medical professional promptly. Avoid unregistered “enhancement” products sold online or at markets, as SAHPRA has warned that some contain hidden ingredients.

Seek urgent medical care for chest pain, an erection lasting more than four hours, sudden weakness or other sudden symptoms.

Consultation price

A consultation is R2,500, in person or remote, and your naturopath confirms any further costs before you go ahead. See our men’s clinic prices for details.
