Varicocele: symptoms, fertility effects and natural support

A varicocele is a swelling of the veins in the scrotum, similar to varicose veins in the legs. It is common, often harmless, but sometimes linked with pain and fertility problems. Here is what to know about varicocele natural treatment and when medical care is needed.

In short: A varicocele affects about 15 percent of men, usually on the left side. It can feel like a “bag of worms” above the testicle and may cause a dull ache that worsens with standing. Varicoceles can raise scrotal temperature and are a common cause of reduced sperm quality. Supportive underwear and lifestyle steps help comfort, but treatment may be needed for pain or infertility.

What is a varicocele?

Veins in the scrotum (the pampiniform plexus) carry blood away from the testicles. If valves in these veins do not work well, blood pools and the veins enlarge.

Symptoms

  • A soft lump above the testicle that feels like a bag of worms
  • A dull ache or heaviness, worse after standing or exercise and better lying down
  • A smaller testicle on the affected side
  • Many men have no symptoms

Effects on fertility and testosterone

Varicoceles can raise the temperature around the testicles and expose them to oxidative stress, which may reduce sperm count, movement and shape. They are found in a large share of men with fertility problems. Some studies link varicoceles with lower testosterone. See signs of male infertility.

When to get checked

See an appropriately registered medical professional if:

  • You notice a new lump or swelling in the scrotum
  • A varicocele appears suddenly, especially on the right side, or does not shrink when lying down
  • You have ongoing pain
  • You and your partner are having trouble conceiving

Medical treatment (surgery or embolisation) may be recommended for pain, testicle shrinkage or infertility.

Natural support for comfort and sperm health

  • Supportive underwear or an athletic supporter reduces aching.
  • Avoid long periods of standing and take breaks to lie down.
  • Keep the testicles cool: avoid hot baths, saunas and laptops on your lap.
  • Antioxidant-rich diet: fruit, vegetables, nuts and seeds. See walnuts benefits for men.
  • Avoid constipation and straining, which raise abdominal pressure.
  • Do not smoke, as smoking further harms sperm.

Some small studies suggest antioxidant supplements may help sperm quality in men with varicocele, but evidence is limited.
