Hydrocele in men: causes, symptoms and treatment options
A swollen scrotum can be worrying. One common cause is a hydrocele, a build-up of fluid around the testicle. Here is what to know about hydrocele in men.
In short: A hydrocele is a collection of fluid in the sac around a testicle, causing painless swelling of the scrotum. In adults it may follow injury, infection or inflammation, or have no clear cause. It is usually harmless, but any scrotal swelling should be checked to rule out other causes. Large or uncomfortable hydroceles can be treated with minor surgery.
What causes a hydrocele?
- Fluid build-up in the membrane around the testicle
- Injury to the scrotum
- Infection or inflammation, such as epididymitis. See epididymitis.
- After groin surgery
- Rarely, a testicular tumour
Symptoms
- Painless swelling of one or both sides of the scrotum
- A feeling of heaviness
- Swelling that may be larger later in the day
- Discomfort from size in larger hydroceles
Diagnosis
An appropriately registered medical professional will examine you and may shine a light through the scrotum (fluid lets light pass). An ultrasound scan confirms the diagnosis and checks the testicle.
Treatment
- Small hydroceles that cause no symptoms are often watched.
- Larger or uncomfortable hydroceles may be treated with minor surgery (hydrocelectomy).
- Draining with a needle is less common because fluid often returns.
When to get checked
- Any new scrotal swelling
- Swelling with pain, fever or redness
- A lump in the testicle. See testicular self-exam.
Sudden severe pain is an emergency. See testicular pain causes.
Comfort tips
- Wear supportive underwear.
- Avoid heavy lifting if uncomfortable.
