Pearly penile papules: what they are and why they are harmless

Many men discover small bumps around the head of the penis and immediately fear an STI. In most cases these are pearly penile papules, a normal skin feature.

In short: Pearly penile papules are small, smooth, dome-shaped or finger-like bumps arranged in one or more rows around the rim of the head of the penis. They affect an estimated 14 to 48 percent of men, are not infectious, not caused by sex and not linked to hygiene. They do not need treatment. Home removal can cause scarring and infection.

What do pearly penile papules look like?

  • Tiny bumps, 1 to 3 mm, white, pink or skin-coloured
  • Arranged in neat rows around the corona, the rim of the glans
  • Smooth, painless and not itchy
  • Usually first noticed in the late teens or 20s

What causes them?

The exact cause is unknown. They are thought to be a normal variation of skin, possibly a leftover from development. They are not caused by an infection, poor hygiene or sexual activity.

Are they contagious?

No. They cannot be passed to a partner.

Pearly papules vs genital warts

  • Papules are uniform in size and arranged in neat rows; warts are irregular and can appear anywhere
  • Papules stay the same; warts may grow, spread or cluster
  • Papules are smooth; warts may have a rough, cauliflower-like surface

If you are unsure, have them checked. See white spots on the penis and small bumps on penis causes.

Do they go away?

They often become less noticeable with age.

Should they be removed?

Treatment is not medically needed. Some men choose removal for cosmetic reasons through an appropriately registered medical professional. Do not try toothpaste, acids, wart removers or cutting at home; these can cause burns, scarring and infection.

The emotional side

Many men feel anxious or embarrassed and avoid intimacy. Knowing that papules are common and harmless helps. Partners rarely notice or mind.
