Small bumps on the penis: common harmless causes and when to worry
Noticing small bumps on the penis can cause a lot of worry. Many are completely harmless, but some need testing. Here is how to tell the difference.
In short: Common harmless bumps include pearly penile papules (small dome-shaped bumps in rows around the head), Fordyce spots (tiny yellow-white spots), and ingrown hairs or folliculitis at the base. Bumps that are new, growing, painful, blistering, ulcerated or cauliflower-like may be genital warts, herpes, molluscum or another infection and should be checked.
Harmless causes
Pearly penile papules
Small, smooth, skin-coloured or white bumps arranged in one or more rows around the rim of the penis head. They are common, not contagious, not caused by sex and need no treatment.
Fordyce spots
Tiny pale yellow or white spots on the shaft or scrotum. They are visible oil glands, harmless and not contagious.
Ingrown hairs and folliculitis
Red or pus-filled bumps at the base of the penis or pubic area, often after shaving.
Sebaceous cysts
Small, firm lumps under the skin, usually painless.
Angiokeratomas
Small dark red or purple spots on the scrotum, common with age.
Bumps that need testing
Genital warts
Flesh-coloured, cauliflower-like growths caused by HPV. See genital warts treatment in males.
Herpes
Painful clusters of blisters that burst and form sores, often with tingling first.
Molluscum contagiosum
Small, shiny, dome-shaped bumps with a central dimple.
Syphilis
A painless, firm ulcer (chancre).
Lichen planus and other skin conditions
When to get checked
See an appropriately registered medical professional or sexual health clinic if bumps:
- Are new or changing
- Are painful, itchy or blistering
- Bleed or ulcerate
- Look like warts
- Appear after a new sexual partner
Avoid sex until you know the cause. See STI symptoms in men.
Do not try to remove bumps yourself
Picking, squeezing or using wart removers meant for hands can damage sensitive skin.

Your health and safety
Our naturopaths use natural remedies and lifestyle support, and always check your health history and current medicines first. Some bumps are caused by infections that need testing and medical treatment, so we refer you to an appropriately registered medical professional whenever needed. Avoid unregistered “enhancement” products sold online or at markets, as SAHPRA has warned that some contain hidden ingredients.
Seek urgent medical care for chest pain, an erection lasting more than four hours, sudden weakness or other sudden symptoms.
