One testicle bigger than the other: what is normal

Most men have one testicle bigger than the other or hanging lower. Usually this is completely normal, but a new difference in size can be a sign to get checked.

In short: It is normal for testicles to differ slightly in size, and for one, usually the left, to hang lower. A new or growing difference, a lump, heaviness or pain may be caused by a hydrocele, varicocele, epididymitis, a cyst, injury or, less commonly, testicular cancer. Regular self-checks help you notice changes early.

What is normal

  • The right testicle is often slightly larger
  • The left often hangs a little lower
  • Small differences have usually been there since puberty
  • Both should feel smooth, firm and similar in texture

Causes of a change in size

Hydrocele

Fluid around a testicle makes one side look swollen. Usually painless. See hydrocele in men.

Varicocele

Swollen veins, usually on the left, that feel like a bag of worms. See varicocele natural treatment.

Epididymitis

Inflammation of the tube behind the testicle, causing pain and swelling. Read epididymitis causes and recovery.

Cysts

Small, smooth lumps above or behind the testicle, usually harmless.

Injury

Bruising and swelling after a knock.

Testicular cancer

Most common in men aged 15 to 49. Often a painless lump or hardness, or a change in size or weight. Early detection makes a big difference.

Torsion

Sudden, severe pain with swelling is an emergency.

How to check your testicles

Once a month, after a warm shower:

  • Hold each testicle between thumb and fingers and roll gently
  • Feel for lumps, hard areas or changes in size
  • Find the epididymis at the back so you know it is normal

Full guide: testicular self-exam.

When to see someone

See an appropriately registered medical professional if you notice:

  • A new lump or hardness
  • A testicle getting bigger or smaller
  • Heaviness, aching or pain
  • Sudden severe pain, which needs emergency care
