In short: Fordyce spots on the penis are small, painless, pale yellow or white bumps caused by visible oil glands. They are harmless, not contagious and not an STI. Any spot that is painful, growing, bleeding, ulcerated or changing should still be checked.

What are Fordyce spots?

Fordyce spots are visible sebaceous (oil) glands. Most oil glands sit around hair follicles, but Fordyce spots appear on skin without hair, so they show up as tiny bumps. They are a normal skin variation. Many adults have them, and they often become more noticeable after puberty.

What do they look like?

  • Tiny bumps, usually 1–3 mm across
  • Pale yellow, white or skin-coloured
  • Often in clusters or rows
  • Found on the shaft of the penis, the scrotum or the lips
  • Painless and not itchy
  • More visible when the skin is stretched, such as during an erection

Are Fordyce spots an STI?

No. Fordyce spots are not caused by infection and cannot be passed to a partner. They are not linked to hygiene or sexual activity.

Fordyce spots vs other bumps

Several harmless and not-so-harmless conditions can look similar. Here is how they usually differ:

  • Pearly penile papules: small dome-shaped bumps in neat rows around the rim of the head of the penis. Also harmless. See pearly penile papules.
  • Genital warts: flesh-coloured, cauliflower-like growths that can spread and grow. Caused by HPV and contagious. See genital warts.
  • Molluscum contagiosum: firm, shiny bumps with a small central dip. Contagious.
  • Herpes: painful blisters or sores that break open and crust.
  • Ingrown hairs or pimples: red, tender and often with a hair in the middle.

Our guides to small bumps on the penis and white spots on the penis cover more possibilities.

Do Fordyce spots need treatment?

No treatment is needed for health reasons. They do not turn into anything harmful. Some men want them removed for cosmetic reasons; options such as laser treatment are cosmetic medical procedures and must be done by an appropriately registered medical professional.

What not to do

  • Do not squeeze or pick them – this can cause infection and scarring.
  • Avoid harsh scrubs, bleach or home “removal” creams on genital skin.
  • Avoid steroid creams unless prescribed for a diagnosed condition.

The emotional side

Many men worry that spots on the penis mean an STI or that a partner will judge them. That worry is understandable. Knowing that Fordyce spots are normal and harmless can take away a lot of anxiety.

When to get a spot checked

See a registered medical professional or sexual health clinic if a spot:

  • is painful, itchy or bleeding
  • forms a blister, sore or ulcer
  • grows, spreads or changes shape or colour
  • appeared after a new sexual partner
  • comes with discharge, burning when you pass urine or swollen glands

If you are unsure, get it checked. Peace of mind is worth it, and regular STI screening is a healthy habit for sexually active men. See STI symptoms in men.
