Gym and hormones
Creatine and testosterone: does creatine affect hormones or hair loss?
Creatine is one of the most popular gym supplements in South Africa. Men often ask about creatine and testosterone, and whether it causes hair loss. Here is what the research shows.
In short: Creatine is one of the most researched sports supplements and reliably improves strength and power. Reviews of studies found it does not meaningfully change testosterone. One small study found creatine raised DHT, a hormone linked with hair loss, but this has not been confirmed. Creatine is considered safe for healthy men at standard doses.
What is creatine?
Creatine is a natural compound made in the body and found in meat and fish. It helps muscles produce quick energy during short, intense efforts.
Benefits
- Increased strength and power
- More muscle when combined with strength training
- Possible benefits for brain function, especially in vegetarians
Creatine and testosterone
Systematic reviews found creatine does not significantly change total testosterone. Any small changes seen in individual studies are not consistent.
Creatine and hair loss
A small 2009 study in rugby players found creatine increased DHT, the hormone that drives male pattern baldness in genetically prone men. Hair loss itself was not measured, and the finding has not been replicated. If you have a strong family history of hair loss and are concerned, discuss it with a professional. See testosterone and hair loss.
Is creatine safe?
- Studies show creatine monohydrate is safe for healthy adults at 3 to 5 g a day.
- It can cause water retention and slight weight gain.
- Men with kidney disease should avoid it unless advised.
- Creatine can raise creatinine on blood tests, which is not a sign of kidney damage in healthy people but should be mentioned when tested.
Creatine vs steroids
Creatine is not a hormone and does not shut down your own testosterone, unlike anabolic steroids. See steroids and erectile dysfunction.
How to take it
- 3 to 5 g a day of creatine monohydrate, any time of day.
- A loading phase is optional.
- Drink enough water.
