Hard flaccid syndrome: symptoms, causes and support
Hard flaccid syndrome is a little-known condition discussed widely in online forums. Men describe a penis that feels firm, rubbery and shrunken when soft, often with weaker erections.
In short: Hard flaccid syndrome is a poorly understood condition in which the soft penis feels semi-firm and shrunken, often with reduced sensation, weaker erections, pelvic discomfort and anxiety. It often starts after an injury or aggressive masturbation or stretching. Pelvic floor tension and nerve sensitivity are thought to play a part. Pelvic floor relaxation, stress management and gentle care help many men.
Common symptoms
- A soft penis that feels firm, rubbery or contracted
- Reduced sensitivity or numbness
- Weaker or shorter-lasting erections
- Pain or discomfort in the pelvis, perineum or penis
- A cold or pale penis
- Anxiety and low mood
What may cause it
Research is limited. Many men report it began after:
- A bending injury during sex or masturbation
- Very aggressive masturbation
- Stretching devices or jelqing
Experts believe overactive pelvic floor muscles and nerve sensitivity, made worse by anxiety, are involved.
What may help
Pelvic floor relaxation
Unlike kegels, the goal is to relax tight muscles. Reverse kegels and slow belly breathing help. See premature ejaculation exercises for reverse kegels.
Stop aggravating activities
Pause jelqing, stretching and rough masturbation.
Manage stress
Anxiety tightens the pelvic floor. See breathing techniques.
Pelvic floor physiotherapy
A pelvic health physiotherapist can assess muscle tension.
When to see someone urgently
Sudden pain, swelling, bruising or a snapping sound during sex may be a penile fracture and needs emergency care.

Does hard flaccid cause erectile dysfunction?
It is often linked to weaker or shorter-lasting erections.
