An itchy penis is uncomfortable and often embarrassing, but it is very common and usually treatable. Here are the most likely itchy penis causes, and how to tell when you need a test.

In short: Common causes of an itchy penis include thrush (yeast infection), balanitis, irritation from soaps or condoms, dry skin, jock itch, pubic lice and some sexually transmitted infections. Gentle washing, avoiding irritants and keeping the area dry help mild cases. Itching with discharge, sores, a rash that spreads or pain on urination should be tested.

Common causes

Thrush (candida)

A yeast infection can cause itching, redness, a white discharge under the foreskin and a burning feeling. It is more common in uncircumcised men, men with diabetes and after antibiotics.

Balanitis

Inflammation of the head of the penis, often from poor hygiene, thrush or irritation. See balanitis.

Contact dermatitis

Soaps, shower gels, washing powder, lubricants, spermicides or latex condoms can irritate sensitive skin.

Dry skin and eczema

Over-washing or harsh products can dry the skin and cause itching.

Jock itch (tinea cruris)

A fungal infection of the groin that causes an itchy, red rash, often spreading to the inner thighs.

Pubic lice and scabies

Intense itching, especially at night, with tiny bites or visible lice.

Sexually transmitted infections

Trichomoniasis, chlamydia, gonorrhoea, herpes and genital warts can cause itching, discharge, sores or bumps. See genital warts treatment in males.

Gentle relief for mild itching

  • Wash once a day with warm water and a mild, unscented cleanser. See penile hygiene tips.
  • Pull back the foreskin to wash and dry gently, then return it.
  • Dry thoroughly and wear loose cotton underwear.
  • Switch to unscented, gentle products and non-latex condoms if latex irritates you.
  • Avoid scratching, which can break the skin.

When to get tested

See an appropriately registered medical professional or sexual health clinic if you have:

  • Discharge from the penis
  • Sores, blisters, bumps or warts
  • Pain or burning when urinating
  • A rash that spreads or does not settle within a week
  • A new sexual partner and new symptoms
  • Recurrent thrush, which can be a sign of diabetes

Avoid sex until you know the cause, to protect your partner.

Private self-check: 5 quick questions

Answer these honestly before you try anything new or book. They help you describe what is happening and get more from your first consultation.

  1. Do you have pain, swelling, redness or itching in the genital area?
  2. Have you noticed discharge, sores, a lump or blood in your urine or semen?
  3. Do you have a fever or feel generally unwell?
  4. Has this lasted more than a few days, or keep coming back?
  5. Is it affecting your sex life, sleep or confidence?

A yes to questions two or three, or sudden severe pain, means you should see an appropriately registered medical professional promptly. For recurring or lifestyle-related concerns, a naturopath can help with natural support alongside medical care.

What happens at your first consultation

  1. Walk in or book online, call +27 10 205 9855 or WhatsApp +27 81 823 1313. In Gauteng, walk in at 199 Vanessa Street, Buccleuch, Sandton, where parking is available. Outside Gauteng, choose a confidential phone or video consultation. Enquiries are answered 24/7.
  2. Talk privately with a naturopath about your symptoms, health history, lifestyle and goals.
  3. Receive a personalised natural plan, with any remedy costs confirmed upfront.
  4. Follow up in person or remotely so your progress is reviewed and your plan adjusted.

Your health and safety

Our naturopaths use natural remedies and lifestyle support, and always check your health history and current medicines first. Itching can be caused by infections that need testing and medical treatment, so we refer you to an appropriately registered medical professional whenever symptoms suggest it. Avoid unregistered “enhancement” products sold online or at markets, as SAHPRA has warned that some contain hidden ingredients.

Seek urgent medical care for chest pain, an erection lasting more than four hours, sudden weakness or other sudden symptoms.

Consultation price

A consultation is R2,500, in person or remote, and your naturopath confirms any further costs before you go ahead. See our men’s clinic prices for details.
