Sperm Leakage During Urination: Causes and When to Worry
Noticing sperm leakage during urination or when straining? Learn the common harmless causes, what can be linked to the prostate, and when to get checked.
In short: Sperm leakage during urination – a small amount of milky fluid at the end of urinating or when straining on the toilet – is usually harmless. It often happens after a long gap without ejaculation or with pelvic pressure. Pain, burning, blood, discharge or cloudy urine should be checked.
What men usually notice
Some men see a few drops of thick or milky fluid at the tip of the penis after passing urine, or when straining during a bowel movement. Others notice a damp spot in their underwear. Understandably, this can be worrying – but in most cases it is not a sign of illness and does not mean you are “losing” your fertility or strength.
Is it really sperm?
Not always. Several fluids can appear at the tip of the penis:
- Semen left in the urethra after ejaculation, flushed out the next time you urinate
- Prostate fluid, pushed out when straining puts pressure on the prostate
- Pre-ejaculate, a clear fluid released during arousal (see what causes pre-ejaculation)
- Discharge from an infection, which is usually yellow, green or cloudy and may smell
Common, usually harmless causes
Long gaps without ejaculation
When you haven’t ejaculated for a while, semen can build up and some may be released during urination, straining or sleep. Wet dreams are a similar process – see nightfall and wet dreams.
Straining on the toilet
Constipation and hard straining press on the prostate and seminal vesicles, squeezing out a little fluid.
Leftover semen after sex or masturbation
A small amount can stay in the urethra and come out the next time you pass urine.
Arousal during the day
Sexual thoughts can trigger pre-ejaculate even without an erection.
Causes that need a check
Prostate inflammation (prostatitis)
Prostatitis can cause leakage along with pelvic pain, burning when urinating, painful ejaculation or frequent urination. See prostatitis.
Sexually transmitted infections
Discharge that is coloured, smelly or comes with burning may be an STI. See STI symptoms in men and get tested.
Urinary tract infection
Cloudy urine, burning and urgency can point to a UTI in men.
Retrograde ejaculation
If little or no semen comes out when you climax but urine looks cloudy afterwards, semen may be going backwards into the bladder. This needs assessment by a registered medical professional.
Does sperm leakage cause weakness or infertility?
This is a common worry, but occasional leakage does not drain your energy, cause weight loss or reduce fertility. Your body makes sperm continuously. Read can you run out of sperm? for more.
What can help
- Avoid constipation: more fibre, water and movement means less straining.
- Keep a regular rhythm: regular ejaculation reduces build-up.
- Pelvic floor exercises: Kegel exercises for men can improve control and help clear the urethra after urinating.
- Manage stress: worry about leakage can increase pelvic tension.
- Wear breathable cotton underwear and keep the area clean and dry.

Frequently asked questions
Is it normal for sperm to come out when urinating?
A small amount of semen or prostate fluid at the end of urinating, especially after a long gap without ejaculation or when straining, is usually normal.
Does sperm leakage cause weakness?
No. Occasional leakage does not cause weakness, weight loss or loss of energy. The body makes sperm continuously.
Can sperm leakage affect fertility?
Occasional leakage does not reduce fertility. If you are trying for a baby and have concerns, a semen analysis through an appropriately registered medical professional can help.
Why does fluid come out when I strain on the toilet?
Straining presses on the prostate and seminal vesicles and can squeeze out a little fluid. Treating constipation usually helps.
When is penis discharge a concern?
See someone if discharge is yellow, green or cloudy, smells, comes with burning or pain, or contains blood.
How much does a consultation cost?
A consultation is R2,500, and any remedy costs are confirmed with you before you go ahead.
