In short: Many men with fertility problems have no obvious symptoms, and the main sign is not conceiving after 12 months of regular unprotected sex. Other signs can include low drive, erection or ejaculation problems, testicular pain or swelling, reduced body hair and breast tissue growth. A semen analysis is the key test.

The main sign

Not conceiving after one year of regular unprotected sex (or six months if your partner is over 35) is the most common sign. Both partners should be assessed.

Possible signs in men

  • Low sex drive or erection difficulties
  • Problems with ejaculation or very low semen volume. See how to increase semen volume naturally.
  • Pain, swelling or a lump in the testicles. See varicocele.
  • Small or firm testicles
  • Reduced facial or body hair
  • Breast tissue growth. See gynecomastia causes.
  • A history of undescended testicles, mumps after puberty, or groin surgery

Common causes

  • Varicocele
  • Infections such as STIs or mumps orchitis
  • Hormone problems, including low testosterone
  • Anabolic steroid use. See steroids and erectile dysfunction.
  • Smoking, heavy alcohol and recreational drugs
  • Excess weight
  • Heat exposure
  • Some medicines
  • Genetic conditions

Semen analysis

A semen analysis checks volume, sperm count, movement (motility) and shape (morphology). An appropriately registered medical professional or fertility clinic can arrange it. Results vary, so a second test is often needed. See what causes low sperm count in men.

Natural steps that support fertility

Sperm take about 2 to 3 months to develop, so give changes at least three months.

  1. Have you noticed a change in the amount, colour or consistency of your semen?
  2. Do you feel pain or burning during or after ejaculation?
  3. Have you had trouble ejaculating, or does it take much longer than before?
  4. Do you smoke, drink most days, or sit for long hours?
  5. Are you and your partner trying for a baby?

Pain, burning, blood in the semen or a sudden change should be checked by an appropriately registered medical professional first. For lifestyle-related changes, a naturopath can help you build a natural plan to support healthy function.

Your health and safety

If anything needs medical attention we tell you honestly and refer you to an appropriately registered medical professional. Avoid unregistered “enhancement” products sold online or at markets, as SAHPRA has warned that some contain hidden ingredients.

Seek urgent medical care for chest pain, an erection lasting more than four hours, sudden weakness or other sudden symptoms.
