Smegma: What It Is, Causes and How to Clean It
Smegma is a white build-up under the foreskin. Learn what causes it, how to clean it gently, when it signals infection, and how to prevent odour.
In short: Smegma is a natural mix of skin cells, oils and moisture that collects under the foreskin. A little is normal. Too much build-up can smell and cause irritation or infection. Gentle daily washing with warm water usually prevents it.
What is smegma?
Smegma is a white or yellowish, cheesy substance that forms under the foreskin of uncircumcised men and around the head of the penis. It is made from shed skin cells, natural oils and moisture. In small amounts it helps keep the head of the penis moist.
Why it builds up
- Not washing under the foreskin regularly
- Heat and sweating
- A tight foreskin that is hard to pull back
- Skin conditions or infections
Is smegma a sign of an STI?
No. Smegma on its own is not an infection and is not sexually transmitted. However, a discharge with a strong smell, colour change, pain or redness may be an infection and should be checked.
How to clean it
1. Gently pull back the foreskin as far as it comfortably goes – never force it.
2. Rinse the head of the penis and the inside of the foreskin with warm water.
3. Use a mild, unperfumed soap if needed, then rinse well.
4. Dry gently and fully.
5. Return the foreskin to its normal position.
Do this once a day. Avoid scrubbing, harsh soaps, wet wipes with fragrance, antiseptics or talcum powder.
When smegma signals a problem
See an appropriately registered medical professional if you notice:
- Redness, swelling, itching or pain – see balanitis
- A white lumpy discharge with itching – see thrush in men
- A strong or fishy smell that doesn’t go after washing – see penis smell causes
- Difficulty pulling back the foreskin – see phimosis
- A foreskin stuck behind the head – this is an emergency; see paraphimosis
Smegma and confidence
Many men feel embarrassed about smegma or odour. It is a normal part of having a foreskin and is easy to manage with simple daily care. Good hygiene also makes intimacy more comfortable for both partners.
Prevention tips
- Wash daily and after sex
- Wear breathable cotton underwear
- Change out of sweaty clothes after exercise
- Dry thoroughly after showering
