Can a Hernia Cause Erectile Dysfunction?
Can an inguinal hernia cause erectile dysfunction or groin pain during sex? Learn how hernias affect sexual health, warning signs, and when to see someone.
In short: Can a hernia cause erectile dysfunction? A hernia does not usually affect erections directly. But groin pain, discomfort during sex, anxiety and, rarely, nerve irritation can make erections and intimacy harder. A painful, hard or stuck bulge needs urgent care.
What is a hernia?
A hernia happens when part of the bowel or fatty tissue pushes through a weak spot in the muscle wall. In men, the most common type is an inguinal hernia in the groin. It often shows as a bulge that gets bigger when you stand, cough or strain, and may extend into the scrotum.
Does a hernia affect erections?
The nerves and blood vessels that control erections are separate from the groin canal, so a hernia rarely affects erections directly. Most men with a hernia have normal erections. However, a hernia can still affect your sex life in other ways.
How a hernia can affect sexual health
Pain and discomfort
A dragging ache or sharp pain in the groin – especially with movement – can make sex uncomfortable and distracting. Pain is one of the most common reasons men avoid intimacy.
Scrotal swelling
A large hernia that extends into the scrotum can cause swelling and heaviness. Other causes of scrotal swelling include a hydrocele and a varicocele.
Anxiety and self-consciousness
Worry about the bulge, or about making it worse during sex, can trigger performance anxiety and softer erections.
Nerve irritation
Rarely, a hernia or the tissues around it can irritate nerves in the groin, causing pain that spreads to the testicles or penis.
What about hernia repair?
Many men have hernias repaired. After surgery, some notice temporary groin discomfort, and a small number experience longer-term groin pain or pain on ejaculation. Your surgeon is the right person to discuss these risks. Erection problems after repair are uncommon.
Is sex safe with a hernia?
For most men with a small, painless hernia, sex is not harmful. Positions that put less strain on the abdomen can be more comfortable. Stop if you feel pain and speak to a registered medical professional.
Warning signs: get urgent care
Go to an emergency department straight away if:
- The bulge becomes hard, very painful or cannot be pushed back
- It turns red, purple or dark
- You have vomiting, fever, or cannot pass wind or stool
These can be signs of a strangulated hernia, which is a medical emergency.
Other causes of groin pain and erection problems
Groin pain with sexual difficulties can also come from prostatitis, pelvic floor dysfunction or testicular pain. An accurate diagnosis matters.
Supporting your recovery naturally
- Avoid heavy lifting and straining until you’ve been assessed
- Treat constipation with fibre and water
- Keep a healthy weight to reduce pressure on the groin
- Stay active with gentle walking
- Pelvic floor exercises may help once cleared – see Kegel exercises for men
