Herpes Symptoms in Men: Signs, Outbreaks and Next Steps
Genital herpes symptoms in men include tingling, blisters and sores. Learn what an outbreak looks like, how herpes spreads, and when to get tested.
In short: Herpes symptoms in men often start with tingling or itching, followed by small blisters that break into painful sores on the penis, scrotum, buttocks or thighs. Many men have mild or no symptoms. Herpes is common, manageable and treatable – get tested if you notice sores.
What is genital herpes?
Genital herpes is a common sexually transmitted infection caused by the herpes simplex virus (HSV-1 or HSV-2). Once you have the virus, it stays in the body and can cause occasional outbreaks. Many people carry it without knowing.
Symptoms in men
First outbreak
The first outbreak is often the most noticeable and may appear 2–12 days after exposure:
- Tingling, itching or burning
- Small, fluid-filled blisters
- Blisters that burst into painful sores, then crust and heal
- Pain when passing urine
- Flu-like symptoms, fever and swollen glands in the groin
Later outbreaks
Recurrent outbreaks are usually milder and shorter. Many men notice warning signs, such as tingling, a day or two before sores appear.
No symptoms
Many men have no symptoms or mistake mild signs for ingrown hairs, friction or a rash.
How herpes spreads
- Skin-to-skin genital contact, including oral sex
- Most easily during an outbreak, but also when there are no visible sores
- Condoms reduce but don’t remove the risk, because the virus can be on skin not covered
Getting tested
An appropriately registered medical professional or sexual health clinic can swab a sore to confirm herpes. Testing is most accurate when sores are present. Public clinics in South Africa offer confidential STI services.
Managing herpes
Prescription antiviral medicines can shorten outbreaks and reduce how often they happen. These are prescribed by a registered medical professional. Supportive self-care during an outbreak:
- Keep the area clean and dry
- Wear loose cotton underwear
- Avoid sex until sores have fully healed
- Pass urine in a warm bath if it stings
- Manage stress and sleep, which may trigger outbreaks
Telling a partner
A herpes diagnosis can feel overwhelming, but it is very common and does not define your relationships. Honest conversations protect partners and build trust.
