Circumcision and sexual health: facts on sensitivity, hygiene and HIV
Circumcision is common in South Africa for cultural, religious and medical reasons. Men often wonder how circumcision and sexual health are connected. Here are the facts.
In short: Research shows circumcision does not consistently reduce sensitivity, sexual satisfaction or ejaculation time. Medical male circumcision reduces the risk of HIV acquisition in heterosexual men by about 60 percent and lowers some other infection risks, but condoms remain essential. Uncircumcised men can maintain excellent hygiene with simple daily care.
Sensitivity and pleasure
Studies comparing circumcised and uncircumcised men, including large trials in Africa, found no consistent differences in sexual satisfaction or erectile function. Some men report changes in sensation, but research does not show a clear overall effect.
Premature ejaculation
Evidence does not show that circumcision reliably causes or treats premature ejaculation. See premature ejaculation myths.
HIV and STI prevention
Three large trials in Africa, including one in South Africa, found medical male circumcision reduced HIV acquisition in heterosexual men by about 60 percent. It may also lower the risk of some other STIs, such as HPV and herpes. It does not provide full protection; condoms, testing and treatment remain essential. See STI symptoms in men.
Hygiene
Circumcision makes cleaning easier and reduces balanitis risk. Uncircumcised men can keep healthy with daily gentle washing under the foreskin. See penile hygiene tips and balanitis.
Medical reasons for circumcision
- Recurrent balanitis
- Tight foreskin (phimosis). See phimosis: tight foreskin in adult men.
- Recurrent foreskin tears. See frenulum breve and foreskin tears.
Traditional circumcision
Traditional initiation is an important cultural practice for many South African communities. Safety measures, hygiene and access to medical care when complications occur are vital.
After circumcision
Sexual activity should wait until healing is complete, usually about six weeks for adults.
