Boxers vs Briefs: Does Underwear Affect Sperm Count?
Does tight underwear lower sperm count? See what research shows about boxers vs briefs, why testicle temperature matters, and how to protect sperm.
In short: Boxers vs briefs and sperm count: research suggests men who usually wear looser underwear such as boxers tend to have higher sperm concentration than men in tight underwear. The likely reason is temperature – sperm develop most efficiently slightly below body temperature.
Why testicle temperature matters
The testicles hang outside the body for a reason: sperm production works most efficiently at a temperature a few degrees lower than the core body temperature. Anything that keeps the scrotum warm for long periods may reduce sperm production. Read more in does heat affect sperm?.
What the research says
A large study of men attending a fertility centre found that men who mainly wore boxers had higher sperm concentration and total sperm count than men who mainly wore tighter styles. The differences were moderate, not dramatic, and most men in tight underwear still had sperm counts in the normal range.
In short: looser underwear may give sperm a small advantage, but underwear alone is unlikely to be the main cause of fertility problems.
Boxers, briefs or boxer-briefs?
- Boxers: loosest fit, most airflow, coolest option.
- Boxer-briefs: a middle ground – choose breathable cotton and a fit that isn’t too tight.
- Briefs: fine for everyday wear, but very tight or synthetic briefs worn all day and night may trap heat.
Comfort and support matter too. During sport, supportive underwear protects the testicles from injury.
Other heat sources to watch
- Laptops on your lap for long periods
- Hot baths, saunas and steam rooms, especially daily
- Sitting for long hours, such as long-distance driving
- Heated car seats
- Tight cycling shorts on long rides (see cycling and erectile dysfunction)
How long does it take to see a difference?
Sperm take roughly two to three months to develop. Any change you make today will show in a semen test after about three months. See how long does sperm take to regenerate.
Bigger factors for sperm health
Underwear is a small piece of the picture. Other factors usually matter more:
- Smoking, vaping and dagga

- Excess weight
- Stress – see can stress cause low sperm count?
- Anabolic steroids and testosterone products, which can shut down sperm production
- Varicocele – see varicocele
- Age – see age and male fertility
